It is a pleasure to write this review of an outstanding and
comprehensive account of contemporary endocrine surgery and
endocrinology. Written by a cadre of experienced Mount Sinai surgeons
and by selected international experts, this text provides a
comprehensive review of the pathophysiology and pathology of surgical
endocrine disease and delineates a thoughtful approach to surgical
therapy.
Endocrine surgery is evolving rapidly and
this text strives to keep us abreast of developments in molecular
endocrinology, imaging, minimally invasive and video-assisted surgery,
and intra-operative assays. The chapters present concise reviews by
organ system (pituitary, thyroid, parathyroid, adrenal, pancreas,
gastrointestinal tract), which emphasize pathology, imaging and surgical
management, in a comprehensive yet readable fashion.
I particularly enjoyed reading the chapter
dealing with the thyroid gland. Thyroid physiology is clearly described
and is correlated with testing of thyroid function and dysfunction,
including hyperthyroidism. Management of thyroid nodules and mediastinal
goiter is presented, as is surgical management of thyroid cancer,
including well-differentiated, medullary and anaplastic cancer. I found
that the majority of problems, which confront the busy thyroid surgeon,
were clearly addressed. The section on MEN syndromes was excellent and
included discussion of the clinical significance of mutation of specific
codons of the RET oncogene. On the other hand, the chapter on the
parathyroid glands, while clearly weighted toward endoscopic and
video-assisted parathyroidectomy for primary hyperparathyroidism,
provided little information on secondary and tertiary
hyperparathyroidism.
Endocrine Surgery provides a practical
summary of diagnostic methods, management options, and surgical
techniques and will benefit surgery residents, general and ENT surgeons,
and endocrine surgeons as they confront the wide variety of surgical
endocrine disorders.
